Adult Foster Care Program and Group Adult Foster Care Program
Bi-monthly nursing and case management visits which can include services such as health monitoring, care supervision and review, assistance with client and caregiver questions, status/wellness checks, home safety checks, ongoing caregiver instruction and direction, medical supply ordering and/or referrals.
Both programs help recipients remain at home with better outcomes, fewer emergency room (ER) visits, fewer hospital admissions and delay or avoidance of long-term care placement. Recipients of AFC and GAFC programs are happier receiving care in their homes while their families can feel more secure and confident as to the overall care and living standard for those family members requiring at least some level of care.
